Celebrate Assist is a program funded and sponsored by Celebrate Vitamins in an effort to assist WLS patients defray the cost of supplementation when faced with financial hardship. This program contains two components. One is designed to assist patients that are dealing with temporary financial issues, and the second is to assist with patients in fixed low-income household. This program is 100% funded by Celebrate Vitamins. It is NOT government sponsored and Celebrate reserves the right to deny assistance to anyone for any reason. 

Here are a couple key points:

• Assistance is not automatic and will be determined based on need. But we will do whatever we can to help.
• Program covers basic supplements that include Multivitamin, Calcium, and Iron. It does not cover protein, bars, or any other accessory items.
• Patients will be required to provide proof of financial status.
• Not all requesting assistance will qualify due to the volume of submissions.
• Once approved, orders must be placed every 3 months by the recipient to ensure account remains in good standing and is not removed from our Assist program.

Fill out the form completely and gather any required documents that are referenced on the form. Submissions that are incomplete will not be processed so be sure that you have everything.
